CANCELLATION POLICY/PATIENT AGREEMENT

We ask that you make every effort to keep your appointment. However, if you are unable to keep your appointment, we kindly ask that you provide our office with 24 hours’ notice. This allows another patient the opportunity to use that appointment time. Furthermore, a fee may be charged if a patient does not show up for an appointment without sufficient notice or if appointment are repeatedly canceled. We greatly appreciate your concern on this matter.
